Home Care Case Coordinator

The Case Coordinator is responsible for managing the various aspects of patient cases, including communication with patients, coordinating with different hospitals, Rehabilitation centers, and ensuring to deliver the highest level of satisfaction between the patient and the service provider. The Case Coordinator works closely with a team of professionals and stakeholders to ensure clients receive the support and services they need. Key Responsibilities: Provide patient with coordinating the services, including intake team coordination, assessment, planning, and documents collection. Coordinate and collaborate with stakeholders, including clients, colleagues, and community partners, to ensure that clients receive comprehensive services and support. Develop and maintain patient records using case notes, progress reports, and other documentation. Monitor patient progress and ensure timelines and goals are met, making adjustments as needed. Participate in team meetings and case reviews to ensure patient's receive coordinated and effective services. Maintain up-to-date knowledge of relevant policies, procedures, and regulations related to case management and patient services. Maintain patient confidentiality related information and adhere to ethical and professional standards. Qualifications: A bachelor's degree in a relevant field such as social work, psychology, nursing, or healthcare administration With 2-3 years of experience in the same field Proficiency in basic computer skills, such as email, word processing, and spreadsheets, is usually required. Should have excellent communication and interpersonal skills and strong organizational and time management skills. Should be able to work under pressure, adapt to changing circumstances, and maintain confidentiality and professionalism at all times.
